Pay: $2,546.40 weekly


Shifts: M-F 8am-5pm; On-call required: Rotating weekends


Seven Healthcare are seeking an experienced RN - Case Management professional in Jeffersonville, IN. This contract role focuses on providing comprehensive case management services for patients, working within a home health environment. The role requires active engagement with members to ensure they receive appropriate care and support.

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement comprehensive care plans.
• Reassess and manage cost-efficient, appropriate levels of care.
• Coordinate care transitions and engage specialty resources as needed.
• Keep detailed records of clinical, functional, and fiscal outcomes.
• Identify opportunities for health promotion and illness prevention.
• Act as patient advocate by protecting privacy and confidentiality issues.
• Engage with interdisciplinary teams including physicians and social workers.

Requirements of the RN - Case Management

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• RN licenses in IN and acceptance of compact licenses.
• Ability to analyze complex medical information and make rational decisions.
• Required home health skills: Wound Care, IV, Drain care, G tubes, basic education.
• Experience with HCHB required.
