Registered Nurse (RN) – Home Health Shift: Monday–Friday, 8:00 AM – 5:00 PM Pay: $59 - $61.50/hour On-Call: Rotating weekends Productivity Requirement: 30 points per week License Required: Active Indiana (IN) RN License or Compact RN License Mileage Reimbursement: $0.43 per mile Coverage Area: Southern Jefferson County and all of Bullitt County Required Skills Home Health experience Homecare Homebase (HCHB) – Required Wound Care IV Therapy Drain Care G-Tube Care Patient Education Primary Responsibilities Perform comprehensive assessments of high-risk patients. Develop and implement individualized care plans in collaboration with primary care providers. Assess and coordinate appropriate, cost-effective levels of care. Arrange non-medical support services (e.g., housing, transportation) to improve treatment compliance. Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals. Utilize specialty resources to optimize patient outcomes. Maintain accurate clinical, functional, and financial documentation. Promote health education and illness prevention. Identify and respond promptly to adverse patient events to minimize complications. Advocate for patients while ensuring privacy and confidentiality. Qualifications Associate's or Bachelor's Degree in Nursing. Current, unrestricted RN license (Indiana or Compact). Strong clinical assessment, critical thinking, and decision-making skills.
