Serves as a subject matter expert in electronic medical records (EMR), and hospice platforms, including SHP. Oversees workflow, including but not limited to admission, discharge, and HOPE. Provides clinical consultation, education, and mentoring to clinical team members regarding best practices and compliance with regulatory requirements. Collaborates effectively with members of the interdisciplinary team to support quality patient care and operational excellence. Utilizes operational data to identify systemic trends, develops recommendations and partners with leadership to implement process improvements What is Required? Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing. Minimum of three to five (5) years’ relevant experience as a registered nurse in a Hospice environment; preferred experience in hospice quality activities. Current State License as a Registered Nurse. Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State. 10% travel. Meets all applicable agency policies and procedures related to health screening and required testing. You will report to the Director of Patient Access.
