Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical conditions, reviewing their medical history and performing necessary assessments to develop tailored treatment plans.
• Communicate evaluation findings and treatment plans to patients and their families, facilitating goal-setting that includes input from all stakeholders to ensure alignment with patient needs.
• Implement evidence-based treatment protocols that adapt to the unique responses of patients, optimizing their recovery process based on ongoing assessments.
• Collaborate with a multidisciplinary team, including physicians and other healthcare professionals, to discuss patient progress and modify treatment plans if necessary, ensuring the highest standard of care.
• Document patient care activities accurately and promptly, maintaining clear and legible records of evaluations, treatment plans, progress notes, and discharge summaries to meet compliance standards.

Patient Population / Client Focus

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid state license to practice physical therapy in California is mandatory.
