Responsibilities:
- You will report to the Associate Medical Director
- You will work fully onsite in Homeland Park, SC
- Provide comprehensive outpatient care for senior patients
- Maintain accurate documentation and medical records
- Diagnose and manage moderately complex conditions
- Coordinate referrals and collaborate with specialists
- Partner with interdisciplinary teams for holistic care
Use Your Skills to Make an Impact
Required Qualification:
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Current national certification (AANP or ANCC)
- Active, unrestricted NP license in SC.
- DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)
Preferred Qualification:
- 2+ years Nurse Practitioner experience
- Experience in primary care, or Medicare Advantage settings with geriatric patients
- Experience with Athena EMR
- Familiarity with HEDIS quality indicators and Medicare guidelines.
Work Environment
- Compensation package including sign-on bonus, relocation assistance, and comprehensive benefits.
- Outpatient clinical setting.
- Standard schedule: Monday–Friday, 8:00 AM – 5:00 PM
Scheduled Weekly Hours: 40
Pay Range: $123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
