Accelerated Home Health Care is seeking a licensed and compassionate Speech-Language Pathologist (SLP) to provide in-home therapy services under physician orders and in coordination with the care team.

 

Key Responsibilities:

• Conduct evaluations and develop individualized treatment plans.
• Implement evidence-based speech and language interventions.
• Educate patients and caregivers on strategies and exercises.
• Document patient progress and communicate with the interdisciplinary team.
• Follow physician orders and all applicable regulations.

 

Minimum Certification & Licensing Requirements:

• Master’s degree in Speech-Language Pathology.
• CCC-SLP certification and state licensure.
• Completion of agency onboarding/credentialing.
• Up-to-date TB test and reliable transportation.

 

Preferred Qualifications:

• Home health experience preferred.
• Strong communication and clinical skills.
• Ability to work independently in a home setting.

 

Working Conditions:

• Travel to patient homes required.
• Flexible scheduling including occasional weekends.
• Physical demands include standing, bending, and assisting patients.
