Community Health Specialist

Position Summary:

The Community Health Specialist will play a key role in expanding the reach and accessibility of the products and tools related to the Association’s Rural Health initiative designated with each state. This position works with the state Community Health Manager and is responsible for identifying, engaging, and onboarding local organizations, health care providers, public health organizations, community health workers, individuals and health systems, including community health centers, rural health clinics, hospitals, and specialty practices to ensure smooth access and adoption of the key resources and tools towards the deliverables of the state specific initiative. Working with other local and state Association staff, the Specialist will guide providers through program onboarding, connect users to training, answer questions, and support ongoing participation of any number of initiatives and services across brain health, care navigation services and supports and workforce enhancement. They will share updates and insights on program performance, impact, and opportunities for improvement as well as support reporting structures. Through proactive communication and support, the Specialist will help seamlessly integrate the rural health initiatives into users' processes and supports to enhance dementia care services in rural settings.

This position is grant funded.

Essential Duties & Responsibilities

• Develop, strengthen, and sustain relationships with rural local health providers, consumers and systems in close collaboration with other local and state teams including the state Community Health Manager to ensure coordinated outreach and avoid duplication of efforts
• Engage in, and report outreach activities, provider engagement, onboarding progress, and activity to internal and external teams on partnership status, impact metrics, trends, challenges, and opportunities for program growth.
• Carry out additional responsibilities as needed to advance the goals of the referral program and the broader rural health strategy including administrative and reporting supports.

Knowledge, Skills and Abilities

• Demonstrated project management skills with the ability to plan, implement and assess a course of action and delivery of results. Comfortable collaborating with internal and external teams, to coordinate outreach and share insights, contacts, and strategy.
• Strong verbal and written communication skills for engaging individuals, community groups, clinicians, administrators, and leaders.
• Understanding of clinical workflows across primary care, hospital, and multi‑specialty settings, with the ability to integrate referral pathways into existing processes. Familiarity with EHR environments and provider operations.
• Ability to work with clarity, patience, and consistent follow‑through, answering questions, resolving challenges, and supporting smooth adoption of rural health initiatives with strong service orientation and responsiveness.
• Comfort working with Excel, CRM systems, or other tracking platforms.
• Skilled at interpreting data and providing insightful updates to internal teams and leadership.
• Ability to manage multiple provider relationships, onboarding projects, and timelines simultaneously. Strong attention to detail and follow‑ Skilled at setting priorities and managing a structured outreach plan.

Qualifications/Education/Experience

• Bachelor’s degree required, preferably in Public Health, Health Administration, Social Work, Nursing, Health Administration, Social Work, Nursing, Public Policy, or a related healthcare or human services field.
• 3-5 years of experience

Position Grade & Compensation: Grade 506 Exempt, The Alzheimer's Association’s good faith expectation salary range for this role is between $63,000.00-$73,000.00
