Patient Navigator - Value-Based Care & Practice Ops

RWJBarnabas Health is seeking a Patient Navigator to support Primary Care practices in achieving improved value-based performance. The role leverages Epic data and population health metrics to identify gaps, develop actionable strategies, and educate practice teams. Travel across Jersey City/Hoboken area is required to coordinate and implement workflow changes.
