Patient Care Associate - Correctional Health Services *Day Tour, 8am-4pm, Various Locations*

(PAGNY) for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. This service provides comprehensive treatment modalities in the jail setting, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, clinics in each facility, substance use treatment, integrated re-entry services, and partnerships with training institutions throughout the city. Responsibilities Facilitate Telehealth encounters as needed Administer total care (bed bath, partial or complete) Assist with patient’s personal hygiene—including skin care, mouth care, shaving, etc. Transfer patients - in and out of bed, stretcher and wheelchair, including physical and mechanical lifting Position patients and carry out range of motion exercises Assist with ambulating and all ADLs as directed Prepare patients for meals and assist with feeding as needed Assist with unit admission and transfer of patient Make beds and tidy patient area for bed-ridden patients Take and document in Electronic Medical Records all vital signs (BP, TPR, weight, intake/output) and perform EKGs as required; submit abnormal EKGs per procedures Administer hot and cold applications to patients under supervision Maintain intake and output for oral and bodily excretions Notify physician/nurse staff of any changes in patient status Perform urine testing by dipstick and fractionation Chaperone staff in clinical areas when requested Maintain competency in nursing skills per clinical Performance checklist for Nursing Assistant Assist with nursing follow-ups and transmit necessary information to laboratory electronically Anticipate and act to maintain a safe environment Promote efficient use of resources in job performance Wait for relief at change of tour by staff with same title Document all encounters in Electronic Medical Records Complete Encounter forms every shift Activities Perform HIV Pre and Post test counseling Perform rapid HIV testing Enter data in the rapid HIV database and EMR Fax rapid HIV testing information to the Nursing Office each tour Perform quality controls Perform EKGs and fax information Perform Nursing Follow-up as assigned Assist with new admission processing Perform venipuncture Maintenance of Unit Clean equipment (thermometers, BP machine, EKG machine, suction, refrigerators, etc.) Stock and replenish supplies in all clinic areas Maintain cleanliness of treatment and exam room surfaces Maintain security precautions with keys and equipment Human Relations Skills Assist other employees as needed Be responsible and cooperative with supervisors, colleagues, patients, visitors, and Department of Correction personnel Maintain a friendly working atmosphere Maintain professional attitude and appearance Adhere to uniform policy and accept constructive criticism Use established channels of communication Respect individuals and recognize limitations; seek assistance appropriately Communicate effectively verbally and in writing Working Conditions Hours include days, nights, weekends and holidays due to round-the-clock care. PCAs may stand for long periods and may need assistance to move or transfer patients. Potential exposure to chemical and infectious hazards. Work with patients who may be confused, irrational, agitated, or uncooperative.
