Full Time, Culver City, CA, US Salary Range: $24.00–$27.00 per hour, DOE Job Title and Details Job Title: Case Manager Department: Full‑Service Partnership (FSP) Program Reports To: Program Manager FLSA Status: Non‑Exempt Work Setting: Primarily Field‑Based Schedule: Full Time, Monday through Friday, 8:30 am to 5:00 pm, including rotating on‑call coverage as assigned Pay Range: $24.00–$27.00 per hour, DOE What You'll Do: The Impact You'll Make As a Case Manager, you will provide community‑based mental health services to adults participating in the Full‑Service Partnership Program. You will support clients through treatment planning, housing placement, care coordination, crisis intervention, rehabilitation services, and community linkage. Client Care and Service Coordination Coordinate medical follow‑up appointments and services for clients with co‑occurring medical and behavioral health conditions. Develop and implement treatment plans in collaboration with clients and multidisciplinary team members. Provide rehabilitation services, life skills training, and support for independent living. Assist clients with locating, securing, and maintaining stable housing placements. Connect clients to community resources and supportive services. Documentation and Compliance Complete accurate, thorough, and timely documentation in accordance with Los Angeles County Department of Mental Health (LACDMH) and Medi‑Cal standards. Maintain required productivity and service utilization expectations. Ensure documentation and service delivery remain compliant with agency and regulatory requirements. Crisis Intervention and Community Support Respond to client crises and provide appropriate intervention and stabilization services. Participate in rotating on‑call responsibilities, including after‑hours crisis response when assigned. Conduct field‑based outreach and engagement activities throughout Los Angeles County. Team Collaboration and Professional Practice Work closely with multidisciplinary team members, service providers, and community partners. Communicate effectively with clients, families, providers, and stakeholders. Maintain professionalism, accountability, and client‑centered service delivery. What You'll Bring: Your Qualifications Education and Experience Bachelor's degree in Psychology, Social Work, Counseling, Human Services, or a related field required. Experience working with individuals living with serious mental illness preferred. Experience with Los Angeles County Department of Mental Health documentation preferred. Experience in community mental health, supportive housing, case management, or related social service settings preferred. Bilingual English/Spanish candidates are strongly encouraged to apply. Skills and Abilities Strong interpersonal, communication, and engagement skills. Ability to work independently in community and field‑based settings. Ability to manage competing priorities and maintain productivity standards. Basic computer proficiency and ability to utilize electronic health record systems. Strong organizational skills and attention to detail. Requirements Valid California Driver License required. Clean driving record required.
