Requirements Locums Eureka, CA Hematology/Oncology *Please read additional details regarding opportunity for an earlier start* ________________________________________________________________ SCHEDULE • Coverage Type: Inpatient and Outpatient • Schedule Type: Clinic and Call • Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call * Please note providers will only be on call 1 out of every 3 weeks. Providers will work regular hours in the clinic and are on call in the evening. It should be billed as regular hours during the day and then night call on weekdays. The weekends are 24hr call. NOTE: physician is expected to respond by phone ONLY and would not be "called back" onsite after hours or on the weekend. No call back should apply! • Dates of Need: Full time need • Shift Times: 8am-12pm, 1pm-5pm • Lunch TBD 1 hour lunch unpaid • Provider Min # Shifts Required: 4 weeks per month • Guaranteed Hours per Day: 8 hours per day • Hours Worked for OT to apply: 10 hours per day (All OT should be preapproved by your clinic manager (Marci D). Please seek preapproval via email with Marci. If you have any questions, please escalate to the PSJH admin team) • Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active only • DEA Required: Yes • Boards: BC or truly BE provider within 3-5 years of residency • Skills/Certs: BLS • Other: Oncology clinic is attached to the chemo treatment center and the hospital, which will require providers to provide two years of inpatient case logs to qualify for privileges. - COC accredited program managing approximately 5oo analytic cases each year Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties. ________________________________________________________________ ADDITIONAL DETAILS If the provider has a clean file, we can pursue temporary privileges for a September start. Providers with settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges.
