Psychiatric Nurse Practitioner / PMHNP - Remote

Primary Care Nurse Practitioner - Manhattan (Bronx)

This Nurse Practitioner will provide accessible, compassionate, trauma-informed primary care directly within the communities where program participants live and spend their time.

The position is approximately 80% community-based, requiring the ability and willingness to travel throughout Manhattan, and occasional Bronx. and provide care in non-traditional healthcare settings.

DUTIES Provide primary and basic medical care to clients in community-based, and field settings. Conduct comprehensive medical assessments, evaluations, and ongoing health monitoring.

Diagnose, treat, and manage common acute and chronic medical conditions within the Nurse Practitioner’s scope of practice. Prescribe and manage medications in accordance with applicable laws, regulations, and organizational policies.

Provide and prescribe medication-assisted treatment (MAT) for individuals with opioid use disorder, as appropriate and within scope of practice. Assess clients' physical health and identify conditions requiring additional evaluation, treatment, or specialty care.

Provide crisis intervention and clinical support during regular business hours. Establish trusting, therapeutic relationships with individuals who may have experienced homelessness, serious mental illness, substance use disorders, trauma, and significant barriers to accessing traditional healthcare.

Meet clients "where they are" and provide person-centered, harm-reduction, trauma-informed, and culturally responsive care.

Coordinate referrals and connections to primary care clinics, specialty providers, hospitals, behavioral health services, substance use treatment programs, and other community resources.

Collaborate with family members, housing providers, shelter staff, hospitals, criminal justice personnel, primary care providers, and other treatment partners, as appropriate.

Participate in interdisciplinary team meetings, clinical case conferences, high-risk case reviews, and care coordination activities.

Work collaboratively with Neighborhood Navigators and other interdisciplinary team members.

Provide care and support to clients participating in other Bridge programs, as needed.

Contribute to quality improvement initiatives and evidence-based practice.

Participate in the development and implementation of clinical policies, procedures, and workflows.

Maintain timely, accurate, and complete clinical documentation in the Electronic Health Record.

Complete required administrative documentation and reports in compliance with payer, funder, organizational, and regulatory requirements.

Support continuity of care and help clients overcome barriers to accessing ongoing medical and behavioral healthcare.

Advocate for clients and assist with connecting them to appropriate medical, behavioral health, substance use, housing, and community resources.

QUALIFICATIONS:

Experience prescribing and managing medications within scope of practice.

Experience working with individuals with serious mental illness and/or co-occurring substance use disorders, and homelessness or housing instability strongly preferred.

Direct or indirect lived experience with, or meaningful connection to, the communities served is highly encouraged.

Must have more than 3,600 hours of qualifying Nurse Practitioner practice experience and be eligible to practice independently in New York State without a written practice agreement with a collaborating physician.

Current New York State Nurse Practitioner license.

Current national board certification in an appropriate Nurse Practitioner specialty.

Must be eligible to practice independently in New York State without a collaborating physician.

Clinical experience in primary care, community health, behavioral health, substance use treatment, or a related setting.

Strong clinical assessment, diagnostic, treatment, documentation, and care coordination skills.

Demonstrated ability to diagnose, treat, and manage common acute and chronic medical conditions.

Experience prescribing and managing medications within scope of practice.

Knowledge of or experience treating individuals with substance use disorders, including opioid use disorder and medication-assisted treatment.

Experience working with individuals experiencing homelessness or housing instability strongly preferred.

Experience working with individuals with serious mental illness and/or co-occurring substance use disorders strongly preferred.

Experience providing community-based, street-based, mobile, outreach, or other non-traditional healthcare services strongly preferred.

Ability to work independently in community and field-based settings while remaining an effective member of an interdisciplinary team.

Strong communication, interpersonal, organizational, and problem-solving skills.

Ability to establish rapport and build trusting relationships with individuals who may be reluctant to engage with traditional healthcare services.

Comfortable working with a high-need population and navigating unpredictable community-based environments.

Flexible and willing to travel throughout assigned Manhattan communities.

Strong commitment to person-centered, harm-reduction, trauma-informed, and culturally responsive care.

Bilingual English/Spanish skills are highly desirable.
