Sign‑On Bonus We are paying a $15,000 sign‑on bonus for this position (subject to successful completion of a 90‑day probationary period – bonus paid in quarterly installments). About the Facility Shore Gardens Rehabilitation and Nursing Center is a premier long‑term skilled nursing facility located in Ocean County, New Jersey. We provide a warm, nurturing environment and aim to maintain the highest quality of life for residents. Position Overview Physical Therapist – evaluates and treats residents, communicates with families, physicians, and other health team members, and maintains documentation. Services include treatment planning, discharge planning, testing abilities, analyzing rehabilitation goals, and consulting with the rehab team. Responsibilities Evaluate residents’ conditions and develop appropriate physical rehabilitation plans. Help residents improve mobility. Relieve pain using modalities, hands‑on therapy, therapeutic exercises, and stretching. Teach residents use of physical therapy equipment. Assist residents with tasks and monitor performance. Maintain confidentiality and protect facility reputation. Comply with federal, state, and local legal and professional requirements. Contribute to team effort. Submit billing logs and necessary information on time. Supervise up to 2 PTAs and cosign all notes. Qualifications Graduate of an accredited physical therapy program. Current New Jersey PT license. 1 year working experience as a physical therapist preferred; new grads considered. SNF experience preferred but not required. Hands‑on experience with Net Health (Rehab Optima) preferred. Current knowledge of treatment practices and drive to learn. Professional behavior. Ability to manage patients with different personalities. Excellent interpersonal communication skills. Additional Information All offers of employment are conditioned on completing and passing a background check, participating in testing requirements (e.g., TB), and using designated PPE when required.
