Home Health Speech Therapist PRN

Position Overview The Speech Therapist (ST) is responsible for developing and implementing individualized care plans to improve client outcomes, coordinating services with physicians and caregivers, and ensuring compliance with licensing and professional standards. The role involves assessing client needs, executing therapy programs, and modifying plans as needed in collaboration with clinical team members. The therapist maintains accurate medical documentation, educates clients and families, and addresses concerns promptly. They work independently within a defined territory, providing therapy services and ensuring effective communication and data transfer. Qualifications & Skills Degree in Speech Therapy from an accredited program Current, unrestricted state license as a Speech Therapist Proficiency in clinical assessments, documentation, and compliance Preferred skills include strong organization, communication, attention to detail, time management, and relationship-building Experience in home health therapy and OASIS documentation is a plus #J-18808-Ljbffr
