Clinical Case Manager (LPN, RN or MSW) - Caldwell ClinicFull Circle Health is looking for a Clinical Case Manager to support our Caldwell Clinic.The purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. Pediatrics experience preferred but not required.Licensure / Certification: Valid license to practice as a Registered Nurse (RN) or LPN or Master's Prepared Social Worker in the state of Idaho, current BCLS certification, and Certified Case Manager certification preferred.Other: Our organization is a tobacco-free workplace.
