Home Health Physical Therapist

Schedule
- Shift: 8-hour days only

Assignment Details
- Patient load: Up to 6 patients per day
- Guaranteed hours: Facility may call off up to one shift per pay period

Requirements
- Required: 1 year of skilled home health experience
- Must hold an active state PT license
- Required certifications: BCLS/BLS
- OASIS documentation experience is explicitly required

Roles & Responsibilities
- Assess and develop individualized treatment plans for patients in home health settings.
- Provide skilled physical therapy interventions to adults and geriatrics, focusing on rehabilitation and mobility enhancement.
- Document patient progress and treatment results in compliance with OASIS standards.
- Educate patients and families on therapeutic exercises and care plans.

Compliance / Notes
- Vaccination: Facility requires primary COVID series plus one booster
- Additional submission items requested to expedite placement:
