Reports To: Administrator / Director of Business Development

Position Summary

We are seeking an energetic, relationship-driven, and results-oriented Community Liaison / Marketing Liaison to represent our Home Health and Hospice Agency throughout the community. The ideal candidate will develop and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, rehabilitation centers, case managers, social workers, discharge planners, and other healthcare professionals.

The Community Liaison will be responsible for promoting agency services, increasing referrals, educating referral sources about home health and hospice care, and strengthening the agency's presence within the community while ensuring compliance with all healthcare regulations.

Essential Duties and Responsibilities

Marketing and Business Development

  • Develop and maintain professional relationships with referral sources including physicians, hospitals, nursing homes, rehabilitation facilities, assisted living facilities, social workers, discharge planners, and community organizations.
  • Promote Home Health and Hospice services to healthcare providers and community partners.
  • Generate and increase patient referrals through ongoing relationship-building activities.
  • Conduct face-to-face visits, presentations, and educational programs for referral sources.
  • Identify and develop new referral opportunities within assigned territories.
  • Attend community events, health fairs, networking meetings, and professional association events.
  • Represent the agency in a professional and positive manner at all times.
Referral Management

  • Educate referral sources regarding agency services, admission criteria, Medicare guidelines, and patient eligibility requirements.
  • Assist referral sources and patients through the referral and admission process.
  • Collaborate with intake staff and clinical teams to ensure timely processing of referrals.
  • Follow up on referral outcomes and maintain communication with referral partners.
Community Outreach

  • Develop strategic marketing plans to grow agency census and market share.
  • Maintain an active presence within the healthcare community.
  • Build partnerships with senior centers, churches, community organizations, and healthcare providers.
  • Promote awareness of home health and hospice services throughout the service area.
Reporting and Documentation

  • Maintain accurate records of marketing activities, referral visits, and business development efforts.
  • Submit weekly activity reports and referral tracking reports.
  • Monitor referral trends and identify opportunities for growth.
  • Assist management in developing marketing strategies and goals.
Compliance

  • Ensure all marketing activities comply with Medicare, Medicaid, HIPAA, and agency policies.
  • Maintain confidentiality of patient and referral source information.
  • Follow ethical healthcare marketing practices at all times.
Education Requirements

Required

  • Associate Degree or Bachelor's Degree in Marketing, Business Administration, Healthcare Administration, Public Relations, Communications, Social Work, Nursing, or a related field.
Preferred

  • Bachelor's Degree in Healthcare Administration, Marketing, Business Administration, or a healthcare-related field.
Experience Requirements

Required

  • Minimum one (1) year of sales, marketing, business development, healthcare outreach, or customer relationship experience.
Preferred

  • Two (2) years or more of Home Health, Hospice, Healthcare Marketing, Physician Relations, or Medical Sales experience.
  • Established relationships within the healthcare community.
  • Experience working with hospitals, skilled nursing facilities, assisted living communities, physicians, and case managers.
  • Knowledge of Medicare and Home Health/Hospice services.
Qualifications

  • Excellent interpersonal and relationship-building skills.
  • Strong verbal and written communication abilities.
  • Professional presentation and public speaking skills.
  • Ability to work independently and meet referral goals.
  • Strong organizational and time-management skills.
  • Ability to analyze referral patterns and market opportunities.
  • Proficient in Microsoft Office and CRM systems.
  • Knowledge of healthcare industry regulations and compliance standards.
  • Self-motivated with a strong customer-service focus.
Licenses and Certifications

  • Valid Texas Driver's License.
  • Reliable transportation with current automobile insurance.
  • Ability to travel throughout assigned territory.
Physical Requirements

  • Frequent travel within the service area.
  • Ability to lift and carry marketing materials up to 25 pounds.
  • Ability to sit, stand, walk, and drive for extended periods.
  • Ability to attend community events and networking functions.

The ideal candidate has healthcare marketing, medical sales, physician relations, or business development experience and enjoys networking, relationship-building, and helping patients access quality care.

Education: Associate or Bachelor's Degree preferred.

Experience: Healthcare Marketing, Home Health, Hospice, Medical Sales, Physician Relations, or Business Development preferred.

License: Valid Driver's License and reliable transportation required.

Schedule: Full-Time

Work Location: Field-Based / Community Outreach
