Position Summary Licensed Practical Nurse II is accountable for the delivery of patient care as delegated by and under the direction of a physician or registered nurse (RN) utilizing the Nursing Process and Relationship‑Based Care model as the framework. The Licensed Practical Nurse II provides direct patient care and takes the initiative to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, and standards of nursing excellence into clinical practice. Through therapeutic relationships with patients, families, self and colleagues, the Licensed Practical Nurse II functions as an interdisciplinary team member who provides quality patient care and fulfills unit and organizational goals. Duties & Responsibilities Complete required orientation program and unit‑specific competencies. Participate in patient assessment as delegated by the registered nurse by observing, collecting, recording, and reporting patient data. Contribute to a plan of nursing care with the registered nurse based on ongoing observations and input from the patient’s family. Implement orders from physician, physician assistant, and/or advanced practice nurse as delegated by the RN and/or physician and perform interventions as prescribed, documenting accurately in the electronic medical record. Share information and collaborate with other members of the health care team to promote effective patient care. Recognize symptoms, establish priorities, and respond to patient’s changing conditions and needs. Engage and obtain information from patient and/or family to support an appropriate patient educational plan. Demonstrate a commitment to UI Health values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment. Conduct screening tests for common diseases and viruses. Advocate for patients, families, and their significant others, the community, and the nursing profession. Integrate theoretical knowledge, evidence‑based research, current standards of practice, and clinical experience to provide quality patient care as measured by established nursing quality standards and other governing/accrediting health care bodies. Recognize and support Shared Leadership as the selected method of nursing governance. Participate in organizational, departmental, and shared leadership activities; support department and unit goals in daily behavior. Demonstrate accountability for job performance by seeking opportunities to enhance job skills and knowledge and maintaining conditions of employment (LPN licensure, BLS and other required unit‑specific certifications). Adhere to the Medical Center’s Code of Conduct, treat patients/families, staff, students and visitors with respect and dignity, and comply with time, overtime and attendance policies. Complete electronic learning modules and related programs as assigned by Medical Center Learning by established date. Comply with University Health Services TB, flu compliance, and fit‑testing requirements. Utilize established communication channels/chain of command to escalor resolve patient care problems. Identify recurring clinical practice and/or related unit problems and participate in problem solving and resolution. Collaborate with other nurses to identify, promote, and provide positive learning experiences for staff and students. Serve as a resource to other nursing staff and students. Access the Medical Center intranet for relevant communication and to review and interpret changes in policy, procedure and other information pertinent to practice. Support nursing and multidisciplinary research efforts conducted on the unit and in the Medical Center. Takes responsibility for continued professional growth/development by identifying learning needs and initiating strategies to meet such needs. Under the direction of a physician or registered nurse (RN), work within the scope of practical nursing as defined by the Illinois Nurse Practice Act. Receive coaching and mentoring from the Director, who provides direction, guidance and feedback regarding practice and performance; meeting on an as‑needed basis. Minimum Qualifications Graduation from an accredited program for Licensed Practical Nurses. Eligibility for licensure as a Licensed Practical Nurse by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. Six months of nursing experience in a health care facility (such as a hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities). Preferred Qualifications Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, families and health care providers. Basic computer skills with the ability to become proficient in Epic with on‑the‑job training.
