Join Our Team as a Charge Nurse (LPN/RN) Rio Grande Rehabilitation & Healthcare Center – La Jara, CO Now Hiring: Charge Nurse (LPN or RN) Rio Grande Rehabilitation & Healthcare Center is looking for a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Charge Nurse . Schedule: Full-Time Rotating weekends What You'll Do: Supervise and coordinate nursing care for residents during your shift. Administer medications and treatments as prescribed. Monitor residents' conditions and promptly communicate changes to physicians and the interdisciplinary team. Provide leadership, guidance, and support to CNAs and other nursing staff. Ensure accurate and timely documentation. Maintain compliance with all state and federal regulations and facility policies. Foster a positive, team-oriented environment focused on quality resident care. Qualifications: Current Colorado LPN or RN license in good standing. Previous skilled nursing or long-term care experience preferred. Strong leadership, communication, and critical thinking skills.
