Licensed Practical Nurse

Group Home Setting Medically Fragile Individuals

$27.36 per hour

This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability.

• • Administer medications, treatments, and tube feedings as prescribed
  • Monitor residents’ health status and promptly report changes or concerns
  • Provide care for individuals with complex medical needs, including:
  • Urostomy care
  • Colostomy care
  • G-tube/J-tube care
  • Catheter care and irrigation's
  • Monitoring vital signs and weights
  • Oxygen therapy, CPAP, and Bi-pap support
  • Educate DSP staff and family members regarding resident care needs
  • Maintain accurate nursing documentation and medication administration records
  • Coordinate medical appointments and communicate with physicians and healthcare providers
  • Ensure compliance with state regulations, infection control standards, and agency policies
  • Respond appropriately and efficiently to medical emergencies
  • Assist with lifting, transfers, and resident mobility support as needed

Qualifications

• Current LPN license in good standing
• Strong clinical and documentation skills
• Experience working with medically fragile individuals, developmental disabilities, or TBI populations preferred
• Knowledge of medication administration and nursing procedures
• Excellent therapeutic communication and interpersonal skills
• Ability to prioritize tasks efficiently and work ethically and professionally
• Ability to work collaboratively within a team-oriented environment
• Compassionate, patient-centered approach to care
