Job Description
Acute Care Case Management - Discharge Planner

Schedule

Weekend requirement: every other weekend. Daily census approximately 25 patients. Must commit to working 3 of the 5 major holidays.

Assignment Details

Orientation: 40 hours. Length of assignment: not specified.


Requirements

• Minimum 2 years of experience in acute care case management or discharge planning in a hospital setting.
• Experience with HMOs/IPAs, utilization review, and care coordination required.
• Ability to independently arrange DME, SNF placement, and ambulance transport.
• Familiarity with CMS, DRG, and Joint Commission/core measures.
• Charting system: Epic.

Responsibilities

• Oversee discharge planning and care coordination for patients from neonates to geriatrics.
• Facilitate family meetings and participate in the treatment planning process.
• Assess benefits and eligibility for care services.
• Coordinate resources for patients facing homelessness, substance use treatment options, and mental health crises.
• Ensure compliance with core measures and quality standards.
