Job Description
Title: LPN - Home Health

Schedule: 8 hours

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- Current Nursing License in the practicing state
- Valid Driver's License
- Current CPR Certification
- Two years of experience as an LPN in a clinical setting, preferably in a home health or hospice setting
- Proficiency in using electronic medical charting and time-keeping software (Homecare Homebase)

Roles & Responsibilities:
- Provide nursing services under the direction of a Registered Nurse, assessing patients' physical, psycho-social, and environmental needs.
- Assist in planning, coordinating, and implementing patient care in collaboration with the RN.
- Communicate effectively with patients, families, and healthcare professionals and document care outcomes appropriately.
- Educate patients and families on self-care techniques and assess their responses to care.
- Maintain infection control measures to ensure safety for both staff and patients.
- Assist with specialized duties related to end-of-life care for hospice patients.
- Support with patient admissions, discharges, and transfers.

Compliance / Notes:
- Must participate in annual in-service and performance improvement programs.
- Reliable transportation and current auto liability insurance coverage are required.
- Must read, write, and speak fluent English.
