Hospice RN Case Manager - Home Health Care Leader

Preceptor Home Health & Hospice is seeking an RN Case Manager to coordinate patient care in Kenosha County. You will develop plans of care, perform assessments, deliver skilled nursing care, and educate families in the home setting. Ideal candidates hold a Wisconsin RN license, have strong clinical and communication skills, and can manage a caseload independently. Home health experience is preferred, but not required.
