Requirements: Proof of current CPR certification; Current state license as a Registered Nurse; Home health experience 2+ years; 1-2 years experience as a Registered Nurse in a clinical care setting; Homecare Homebase Software Experience - Required; OASIS Experience - Required; Reliable transportation and insurance - Required.

Responsibilities:

• Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, and safety, using appropriate teaching methods.
• Provide supervision and clinical directions to home health aides and licensed vocational nurses.
• Perform on-call responsibilities and deliver on-call services as assigned.
