Bilingual Case Manager/QBHS

Community Health Alliance is seeking a Bilingual Case Manager/QBHS who is fluent in both Spanish and English to provide culturally responsive, community-based behavioral health services and support to Spanish-speaking children, adolescents, adults, and families. This role is community based and includes home visits, service coordination and collaboration with providers, families and other supports. Why Join Community Health Alliance? Facilitates healthcare, behavioral health and other services and supports as part of care coordination. Provides training and facilitates linkages for the use of community resources. Monitors symptoms and service delivery to ensure client needs are met. Obtains or assist clients in obtaining necessary services for basic necessities and services. Assists clients in accessing self-help support groups and increasing social support networks. Provides appropriate interventions to aid clients in the restoration of daily living skills. Coordinates and assist in crisis management and stabilization. Collaborates and communicates effectively with treatment team for the coordination of care. Qualifications Required Qualifications High School diploma plus 3 years of direct experience. Associates or Bachelor's degree in a social sciences field or related field is preferred. Spanish/English bilingual. An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior. An understanding of how to therapeutically engage a person with mental illness. Valid driver's license, car insurance, and insurable driving record required. Willingness to travel throughout the assigned service area. Must have reliable transportation. An understanding of the community mental health system.
