LPN Shift Details: 08:00 AM - 04:00 PM, 5 shifts per week, scheduled hours 40. Job order start date: 07/21/2026, end date: 10/20/2026, duration 13 weeks.

Job Description: LPN required. 1.5+ years experience, NY license, experience in rehab nursing center and LTC. Every other weekend. PointClickCare-EMR preferred. At times will be responsible for passing medications to 40 long term care established residents. Local radius 75 miles. Locals accepted at local BR: $7 less BR. Modified time: 7/21/2026 12:00:00 AM.

Job Requirements & Qualifications: Previous charge experience, years of experience, patient ratio experience, PointClickCare charting system preferred. Community hospital experience, LTAC experience, trauma level I/II experience, travel experience required. Certifications required. Skills needed. Unit details: scheduling type, patient ratios, float required, call required, weekend coverage, number of weekend shifts per contract, pre-approved time off, orientation hours.

Facility & Patient Care Details: Patient age groups, daily census, number of visits per day, number of rooms, number of beds, interdisciplinary support, patient diagnoses, special procedures/unit details, special equipment.

Travel Compliance: SSN/DOB RTO - preferred 7 days or less, 1+ years required for submission (within specialty submitting to), travel experience preferred. For CNAs: Travelers with license/BGC hits cannot be accepted due to CHRC process. Submit with recent Nursys. Valid NY State License (must have at least applied if still pending). References required. Certifications will allow travelers to get before starting. Permanent staff cannot return as a traveler within a year. Candidates may float within skillset. Client will not clear without SSN (Canadian Travelers). Please ensure all international travelers apply for Social Security appointment at time of submittal. International travelers cannot cross any sooner than 10 days prior to scheduled start date. Client will not clear with pending SSN or work authorization. Local radius: 75 miles. Local rule: accepted at lower rate. Orientation: Minimum of 3 hours with a 6-hour max. Extensions: After the 3rd assignment they can do 3 more but at the local rate. Modules: Modules are billable. Guaranteed hours: Contract weeks: 91.
