What You'll Do Provide evening hospice nursing visits throughout the Twin Cities Metro. Assess patient needs and provide symptom management, medication administration, and comfort-focused care. Educate and support patients, caregivers, and family members. Deliver care in private homes, assisted living communities, memory care, and skilled nursing facilities. Collaborate with physicians and the interdisciplinary hospice team. Complete timely and accurate EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Experience in hospice, ICU, ER, oncology, med-surg, home health, or palliative care is highly valued. Nurses interested in transitioning into hospice are encouraged to apply. Strong assessment skills and the confidence to work independently. Excellent communication and compassionate patient care.
