THE ROLE

A home care coordinator is often the person who makes the difference between a client having reliable care that day and a shift going uncovered. People Care is hiring three Bilingual Home Care Coordinators to support our home care programs, including DFTA / NYC Aging and NHTD-related coordination. Depending on operational needs and experience, coordinators may maintain a primary case list within one program while also providing backup coverage across the coordination team. This is an execution-focused role. You are responsible for keeping authorized services staffed, maintaining continuity of care, responding quickly to coverage problems, and keeping scheduling and case documentation accurate.

WHAT YOU OWN

Your assigned case list, end to end. Keep authorized clients appropriately staffed, maintain coverage, follow open issues through resolution, and make sure important details do not quietly fall through. Aide-to-client matching. Coordinate matches based on client needs, language, geography, availability, schedule, program requirements, and client preferences while supporting continuity whenever possible. Scheduling and EVV integrity. Maintain accurate scheduling and visit records in HHAeXchange or the applicable system. Monitor exceptions and follow unresolved or unverified visits through the appropriate process. Coverage under pressure. Respond to call-outs, schedule changes, open shifts, and other coverage problems quickly. Escalate issues appropriately when continuity of care is at risk. Program documentation. Maintain current case notes, authorizations, scheduling records, and other required documentation according to the requirements of the program or case population assigned to you. Team coverage. Provide backup support when another coordinator is unavailable or when coverage volume requires additional help. The goal is continuity for clients, not ownership of information by only one person. Communication. Communicate clearly with clients, families, aides, nurses, clinical staff, compliance, billing, and other operational partners and document important follow-up.

WHO YOU ARE

You are organized enough to manage a full case list without losing the small details that matter to a client's care. You identify coverage problems early, communicate them clearly, and stay with an issue until it is appropriately resolved or escalated. You are also comfortable working as part of a coordination team. Although you will have cases and responsibilities you directly own, you can step in when another coordinator or program needs coverage.

REQUIRED QUALIFICATIONS

Bilingual English and Spanish 1+ years of home care coordination, scheduling, or closely related home care operations experience Experience managing multiple client cases, schedules, or service needs at the same time Experience responding to call-outs, coverage gaps, scheduling changes, or other time-sensitive service issues Experience with HHAeXchange or a comparable home care scheduling / EVV system Ability to maintain accurate case notes, schedules, authorizations, and operational documentation Clear professional communication with clients, families, caregivers, and internal teams Ability to work 100% on-site at our Brooklyn office Ability to work effectively within structured workflows, escalation paths, and accountability systems

PROGRAM EXPERIENCE

Experience with one or more of the following is strongly preferred: DFTA / NYC Aging NHTD Medicaid waiver programs MLTC

CDPAP

Other managed long-term-care or home-care programs Direct DFTA or NHTD experience is especially valuable, but candidates with strong home-care coordination experience and transferable program knowledge may also be considered.

When coordination works well, the right caregiver is scheduled, changes are handled quickly, records stay current, and clients experience continuity. When a problem develops, the coordinator is often the person connecting the client, caregiver, clinical team, and operations team to get it resolved. That responsibility matters whether the case is supported through DFTA / NYC Aging, NHTD, or another People Care program.
