Client Care Manager

HomeWell Care Services of Delaware is seeking a dedicated Case Manager to coordinate client services, conduct assessments, and maintain accurate documentation within our Smyrna office. The role ensures high-quality, compliant care and seamless communication with clients, caregivers, and referral partners.
