Job DetailsJob Location: Chester, CA 96020Education Level: NoneSalary Range: $27.68 - $38.66 HourlyJob Shift: Day ShiftJob Summary This position is responsible to assist in the coordination and completion of the day-to-day workload of the Health Information Management (HIM) and Patient Access departments. They assist the department director in developing daily teamwork plans and provide support and input in organizing the distribution of the work and ensuring the quality of registration for all service types occurring in the hospital and clinic. As the subject matter expert, they provide input on performance improvement, overall department development of training programs, and in‑services. They ensure the quality, accuracy, accessibility, and security of paper and electronic record systems. The HIM/Patient Access Lead provides support/assistance to the HIM and Patient Access Director in all aspects of the department activity. Responsibilities Provides coverage when called upon to perform any of the HIM/Patient Access Representative. Provide support in organizing work distribution through effectively managing workload to meet department targeted standards. Monitors and completes quality and accuracy audit activities for Patient Access and HIM. Mentors staff to assure consistency of department operations. Monitors the daily workflow and productivity of staff. Closely monitors and provides guidance to staff to ensure they understand their assigned duties or delegated tasks. Assist in providing training and re‑training to all staff and new hires. Provide support and coordination of team personnel issues, staff reviews, and schedule management. Build positive relationships and inspire others to perform at a higher level. Provide input on improvements in performance of respective service line. Work with department Director and other departments to identify process issues or obstacles. Meet daily productivity requirements based on assigned goals and metrics. Integrates data from a variety of electronic and paper sources. Assist and contribute to the weekly service line team data and performance reporting. Communicate potential risk management issues to Director and other team members. Maintains up-to-date knowledge of applicable regulatory and department policies are met to ensure compliance on an ongoing basis (CMS, CDPH, Title 22, HIPAA, HITECH, OSHA, Safety Standards, etc.). Participate in continuous improvement activities ensuring appropriate tools are deployed and results are evaluated and/or measured. Perform other duties as assigned. HIM Essential Duties Compose and distribute the hospital census daily. Processes paper charts into the EMR and analyze the encounter for deficiencies. Monitors chart completion process, retrospectively and concurrently, and then communicate that information to deficient physicians through the established suspension process. Assist with the collection of data to feed into various QA monitors set up by the department Director. Monitors and coordinates release of information functions, whether to patients or other requestors, assuring timeliness and accuracy. Researches any issues related to billing, by utilizing the appropriate reports necessary that need medical record validation. Understands the retention requirements and be actively involved in directing the storage and destruction of these medical records. Works as directed with the Skilled Nursing management to maintain the thinning and scanning of SNF charts quarterly and post-discharge. Understand the process for forwarding supporting documentation on cancer cases to the California Health Collaborative quarterly. Responsible for reporting the facility deaths to the California Donor Network on a monthly basis. Responsible for training or overseeing the training of new HIM/Patient Access personnel in the basic clerical functions of the department. Able to cross-train and assume oversight of the Clinic Chart Room when called upon to do so. Responsible for overseeing the HIM and Patient Access departments in the absence of HIM/Patient Access Director. Other duties as directed by HIM/Patient Access Director. On-Call duties as required to support the department. Patient Access Essential Duties Obtain correct patient demographic and financial information in order to properly register the patient for services. Explain the basics of the advance directives to patients and be able to guide them through completing the form. Explains the basics of all forms signed by the patient all signatures are captured on all required registration documents. Answers and appropriately routes all incoming telephone calls. Collects co-pays, deductibles, and payments per financial policy. Maintain accurate records of cash, check, and credit card payments. Check for redundant medical records numbers. Always maintain patient confidentiality and privacy. Ability to lead others, organize tasks, and work independently. Ability to establish and maintain effective working relationships with co-workers, physicians, and leaders in other departments. Ability to provide proactive leadership of the department’s processes; ability to direct and coordinate all hospital Patient Access activities according to policy. Responsible for training new registration staff members on how to properly register patients for different services provided at the facility. Assure cross-training is accomplished in the department to provide continuity within the department. Assists in developing and implementing new procedures. Perform all Patient Access related clerical functions, including assembling chart packets per type of service, monitoring standing orders, verifying insurance information, and other duties as assigned. Responsible for overseeing the HIM and Admitting departments in the absence of the HIM/Patient Access Director. Fills-in as required to support the department coverage. On-Call duties as required to support the department. Other duties as directed by HIM/Patient Access Director. QualificationsQualifications Three years minimum of general clerical experience in a HIM and/or Patient Access department preferable. Proficient typing speed at minimum of 35 words per minute with accuracy. Knowledge of medical terminology and anatomy and physiology. Must possess the ability to communicate effectively both orally, and in writing, to include legible handwriting, correct grammar, and spelling. Must possess good working command of the English language. Must be literate in the Microsoft Office Suite. Must possess the ability to operate standard office equipment and be computer literate.
