Position Overview

This position requires a valid California CNA license and involves working closely with nursing staff, residents, families, and other departments within a skilled nursing facility (SNF) to provide individualized care for residents. The role involves various shifts, including days, nights, weekends, and holidays, with a possibility of floating to different units as needed.

Key Responsibilities

• Receive and provide reports on each resident's condition, ensuring a clear understanding of required care.
• Conduct rounds on all residents, including assessments for new and ill residents, as well as those who have fallen or sustained injuries.
• Document assessments according to facility policies.
• Alert nursing staff to any changes in residents' conditions or needs.
• Assist with direct patient care, ensuring prompt response to call lights.
• Foster positive working relationships with other departments, co-workers, residents, families, and physicians.
• Perform other duties as assigned.

Qualifications

• Must possess a valid California CNA license.
• Strong communication skills and the ability to collaborate effectively within a team.
