CNA - LTC/SNF

Job Description

Job Summary:
Provide direct care to residents in a skilled nursing facility, ensuring their comfort, safety, and well-being. Collaborate with healthcare team members to deliver personalized, quality care in a fast-paced environment.

Responsibilities:

• Receive and communicate resident updates accurately to nursing staff.
• Assess residents' conditions during rounds, including new admissions and post-injury visits.
• Document assessments and care provided according to facility policies.
• Notify nurses promptly of any changes in residents' health status or needs.
• Assist residents with activities of daily living and respond quickly to call lights.
• Foster positive relationships with residents, families, co-workers, and physicians.

Qualifications:

• Hold a valid California CNA license.
• Demonstrate excellent communication and teamwork skills.
• Have previous experience working in LTC or SNF settings preferred.
• Possess the ability to work weekends and holidays as required.
• Be adaptable and willing to float to other units as necessary.
