Hospice Admissions & Care Liaison

Gentiva seeks an experienced Hospice Clinical Liaison to coordinate intake, referrals, and admissions, building strong partnerships with hospitals, physicians, discharge planners, and families to ensure timely, compassionate care. The role requires RN licensure with 3–5 years of hospice experience; BSN preferred.
