IRIS Consultant

Lutheran Social Services of WI & UP MI seeks an IRIS Consultant to support adults with disabilities and older adults in self-directed long-term care. In this role, you’ll partner with participants to assess needs, develop person-centered service plans, and help manage IRIS budgets and supports. You’ll coordinate community resources, monitor health and safety, and ensure compliance with program guidelines. Working in a faith-inspired, inclusive, and trauma-informed environment, you will collaborate with interdisciplinary teams, maintain accurate documentation, and advocate for participant choice, independence, and quality of life.
