IRIS Consultant

Lutheran Social Services of WI & UP MI is seeking an IRIS Consultant to support adults with disabilities and elders in self-directed long-term care. In this role, you will guide participants through person-centered planning, assess needs, and help them design and manage IRIS budgets and services that promote independence, safety, and community inclusion. You will conduct home visits, coordinate with providers, ensure program and Medicaid compliance, and maintain accurate documentation. The IRIS Consultant collaborates with families and community partners while upholding LSS’s trauma-informed, faith-inspired, and culturally responsive values.
