Physical Therapist

Key Responsibilities

Provide high-quality physical/occupational therapy care to patients in a home health setting
Conduct Start of Care (SOC), recertifications, and evaluations
Perform treatment visits in accordance with care plans
Accurately document patient care in the EMR system (HCHB preferred)
Develop and implement individualized treatment plans
Collaborate with interdisciplinary healthcare teams
Ensure compliance with clinical, regulatory, and agency standards

Required Qualifications

Active PT or OT license (state-specific)
Minimum 1 year of Home Health (HH) experience
Experience with Homecare Homebase (HCHB) documentation system
