MDS Coordinator

Hudson Elms Nursing & Rehab is now hiring an RN MDS Coordinator! We are currently seeking an experienced and detail-oriented RN MDS Coordinator to join our team. Experience with MDS 3.0, care planning, and RAI processes in a long-term care setting. Knowledge of Medicare/Medicaid regulations and reimbursement guidelines. Excellent organization, communication, and computer skills. RAC-CT certification preferred (or willingness to obtain upon hire). Responsibilities: Complete and coordinate accurate and timely MDS 3.0 assessments in compliance with federal and state regulations. Develop and update individualized care plans in collaboration with the interdisciplinary team. Ensure appropriate documentation supports coding and reimbursement accuracy. Submit MDS assessments and track CMS submissions to ensure error-free processing. Participate in care planning meetings, QA audits, and survey readiness.
