We believe it's worth every effort because no one should face the end of life alone.Join us and be part of a team that stands up for compassion, that listens, that grows, and that ultimately makes a profound difference in our community.Intake Coordinator Qualifications:At least 3 years of hospice experience (intake preferred, billing acceptable) in a high-volume referral settingReceive and promptly process hospice referrals and inquiries, scheduling a hospice representative with the patient and family to assess the need for servicesAssign patients to the appropriate team after admission, in coordination with team leadersReady to apply hospice experience to the workflows of a centralized system; central office experience is a plus, including handing off and receiving incomplete work itemsStrong working knowledge of Teams, SharePoint, and electronic clinical recordsAble to work across multiple systems and juggle competing priorities with strong attention to detailCompassionate, professional phone presenceReliable transportation, a valid driver's license, and the ability to safely operate a vehicleIntake Coordinator Schedule: Tuesday to SaturdayTuesday to Friday: 10:00 AM - 6:30 PMSaturday: 11:00 AM - 7:00 PMIntake Coordinator BenefitsBe part of a team willing to grow, listen, be heard, and be challenged.Health, Dental, Vision, 401k, PTO.Set the industry bar for the standard of care.Continuing Education Reimbursement.Ability to grow into a variety of different roles inside our team and organization.Competitive Industry pay.
