Position: Social Services Designee We are seeking a Social Services Designee to support our residents by ensuring their social and emotional needs are met, coordinating discharges, and participating in care conferences. Responsibilities Assist residents in preparing for their future. Ensure residents receive a high standard of care and support during their stay. Coordinate discharges and participate in resident care conferences. Qualifications Minimum: high school diploma or equivalent and at least 2 years of experience in a Social Worker role, preferably in a healthcare setting. Preferred: Hindi, Urdu and/or Gujarati language skills. Preferred: Prior Social Services Designee experience in a Skilled Nursing Facility.
