About This Role The PRN RN Case Manager at Cleveland Clinic’s Martin North Hospital is responsible for facilitating effective transitional care coordination while working collaboratively with inter-professional teams. Highlights Location: Stuart, FL Facility: Martin North Hospital Schedule: Per Diem, Day shift What You'll Do Ensure compliance with standards of accrediting bodies and state mandates. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community. Proactively assess and mitigate risks to patients’ transitions of care. Work with patients and care partners to develop individualized transitional care plans. Participate and lead in patient rounding activities, huddles, and committee meetings.
