Job Description

** JOB SUMMARY **

This paragraph summarizes the general nature, level and purpose of the job.

As part of the Medical Home Program, provide outreach, training and technical assistance to pediatric providers in private practice and clinic sites in low-income county. Work in collaboration with public health and other child agencies to promote developmental screening and appropriate referrals for services across children and youth populations.

The office for this position is located in Emeryville and job activities are located in Alameda County.

** ESSENTIAL FUNCTIONS **

Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.

  • Maintains appropriate contacts and working partnership with designated County Public Health, pediatric primary care providers and clinics, and public and private agencies to support medical homes, developmental screening, and appropriate referral and follow-up for children and youth.
  • Identifies and outreaches to pediatric primary care practices and clinics in designated County that serve a high volume of low-income children.
  • Trains providers and office staff on medical home concepts, developmental screening, and referral pathways for children and their families.
  • Identifies systems barriers to provision of medical homes and developmental screening and identifies potential solutions.
  • Conducts regular site visits to practice sites to provide technical assistance, additional training, and consultation for pediatric providers and staff on medical home concepts, developmental screening, and referral pathways.
  • Assists in design, revision and distribution of medical home materials and other educational materials for pediatric providers, including updates on program, systems, and policy changes that affect pediatric practice and services to children and families.
  • Participates in the collection, consolidation and assessment of program data for the purpose of monitoring the results and overall effectiveness. May recommend areas of improvement.
  • Prepares project reports, policies, and procedures and disseminates them to senior management for project reports.

Qualifications:
Qualifications

** MINIMUM QUALIFICATIONS **

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education: Bachelor's degree in a work-related discipline/field from an accredited college or university.

Experience: Two (2) years of progressively responsible and directly related work experience; experience with child health and/or development and/or pediatric primary care settings is preferred.

License/Certification: None required.

Pay Range
Compensation is based on the level and requirements of the role.
Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.
Typically, new team members join at the minimum to mid salary range.
Minimum to Midpoint Range (Hourly): $37.62 to $48.91
