Responsibilities

• Provide direct speech-language pathology services to home health patients per the established plan of care.
• Assess, diagnose, and treat communication and swallowing disorders; develop and implement individualized treatment plans; document patient progress.
• Collaborate with an interdisciplinary team to support patient goals and optimize outcomes.
• Educate patients and families on therapy techniques, compensatory strategies, and home exercise programs.
• Maintain accurate, timely documentation in accordance with licensure, certification, and agency policies.
• Participate in ongoing quality improvement and contribute to the comprehensive plan of care.

Requirements

• Active Massachusetts Speech-Language Pathologist license (required)
• Master’s or Doctorate degree in Speech-Language Pathology (required)
• Certificate of Clinical Competence (CCC) (required)
• Minimum 2 years of SLP experience in home care/home health (required)
• Valid driver license and own vehicle (required)
• Ability to work independently with minimal supervision (required)
• Acute care experience is preferred
