Responsibilities
- Conduct OT evaluations and ongoing interventions for residents with acute rehab needs within the SNF setting
- Develop, implement, and adjust individualized treatment plans focused on activities of daily living, mobility, cognition, and safe functional independence
- Reassess progress, document outcomes, and communicate changes to the interdisciplinary care team
- Support discharge planning, caregiver education, and collaboration with nursing, therapy assistants, and case management
- Ensure timely, accurate documentation and compliance with regulatory requirements; maintain patient safety and infection control standards

Requirements
- Licensure, certifications, and education requirements will be provided in the Job Notes and verified during the hiring process
- Ability to work effectively in a skilled nursing facility rehab environment
- Willingness to commit to a 13-week travel assignment in Seattle, WA
- Strong clinical documentation, teamwork, and adherence to facility policies
