Care Liaison Manager: Hospital & Home Health Alignment

Providence at Home with Compassus

Providence at Home with Compassus is seeking a Care Liaison Manager to connect hospitals with Home Health and Hospice, coordinating care, supporting staff, and advancing value-based care goals across the territory. This role leads care coordination activities for each service line, builds essential relationships with Providence hospital systems, and drives a market strategy through JV and ministry leaders. A clinical license or Bachelor’s degree in social work is required, with several years of #J-18808-Ljbffr Providence at Home with Compassus
