Provider would work 5-7 12 hour shifts per week with evening call post shift. For November, we need to cover the following shifts: November 13, 14, 15 (3- 24hours) and ending on November 16th at 7AM. If we can get a new provider credentialed in time, then we would add an orientation shift on November 12th or the 17th 8A to 8P. Pending on their start date. November 18, 19, 20 (3- 24hours) and ending on November 21st at 7AM. Possibly November 29, 30 and December 1 (3- 24hours) and ending on December 2nd at 7AM. For December we need to cover the following shifts, December 5, 6, 7 (3- 24hours) and ending on December 8th at 7AM. December 11,12,13 (3- 24hours) and ending on December 14th at 7AM. December 23, 24(2- 24hours) and ending on December 25th at 7AM. December 30, 31 and January 1 (3- 24hours) and ending on January 2nd at 7AM.
