Occupational Therapist (OT) / Occupational Therapy Assistant (OTA)
Homeside Rehab is seeking compassionate and motivated Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs) to provide one-on-one home-based therapy services in Carle Place, NY 11514 .
At Homeside Rehab, we provide high-quality rehabilitation services in the comfort of our patients' homes. Job ResponsibilitiesProvide skilled Occupational Therapy services in the patient's home
Evaluate patients and develop individualized plans of care, as applicable to your professional scope of practice
Carry out established treatment plans and therapeutic interventions
Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your professional scope
Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team
Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
Communicate professionally with patients, caregivers, therapists, and office staff
Provide one-on-one therapy services in the patient's home
Active New York State Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) license/certification , as applicable
Basic computer proficiency for electronic documentation
Strong interpersonal and communication skills
Ability to work independently in a home-based setting
Valid driver's license and reliable transportation for home visits
Home health, outpatient, rehabilitation, or skilled nursing experience preferred
Minimum of 1 year of clinical experience preferred
Job Type: Full-Time, Part-Time, Per Diem
Location: Carle Place, NY 11514
Pay: $45–$70 per hour , based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
