Field-Based Community Health Advocate
AbsoluteCARE Inc
AbsoluteCARE Medical Center & Pharmacy is seeking a Community Health Worker to engage with members across Cleveland communities. You will conduct in-person assessments, develop care plans, and connect members to essential resources to address social needs and barriers to care. The role requires a valid Ohio CHW certification, CPR/BLS, and a willingness to travel locally at least 80%. This position reports to the Community Team Care Manager and the Engagement Director. #J-18808-Ljbffr
