Therapist - Physical Therapist

Job Description

Job Summary:

Provide expert physical therapy services to patients in an outpatient setting, designing and implementing personalized rehabilitation programs to restore and improve physical function. Collaborate with healthcare team members to ensure comprehensive patient care and achieve optimal recovery outcomes.

Responsibilities:

• Develop and adjust individualized Plans of Care based on initial and ongoing patient evaluations.
• Administer a wide range of physical therapy techniques to facilitate mobility, strength, and overall function.
• Supervise and mentor physical therapy assistants to maintain treatment quality and consistency.
• Document patient progress accurately, ensuring compliance with third-party payer requirements and facility policies.
• Educate patients and caregivers on home exercise programs and discharge instructions.
• Communicate effectively with interdisciplinary team members and referral sources to coordinate care.

Qualifications:

• Hold an active Illinois/IL Physical Therapist License.
• Possess at least 1 year of outpatient physical therapy experience.
• Demonstrate proficiency in a variety of physical therapy techniques.
• Provide supervisory references from previous employers.
• Maintain current BLS certification from the American Heart Association.
• Exhibit strong interpersonal and communication skills, with a focus on compassionate patient care.
