Nurse Navigator

This position is for a Nurse Navigator specializing in patient care. The role focuses on working collaboratively with healthcare providers to coordinate patient care. Responsibilities include patient education, follow-up instructions, and medication management. You will manage patient communications through phone calls and messages. Responsibilities also encompass evaluating laboratory results and prioritizing patient care. The Nurse Navigator will be involved in infusion and direct admits by a physician. Experience in medical-surgical settings is required. The ideal candidate should be adept at handling busy phone lines and managing emails. You will work with patients primarily in the adult and geriatric age groups. Close coordination with designated medical providers to optimize patient outcomes is vital. Candidates should be comfortable using electronic medical records. Skills in intravenous infusions, telephone triage, patient teaching, and minor injections are essential. This role requires strong organizational skills and the ability to multitask. A commitment to patient advocacy and high-quality care is expected.
