Medrite Community Care is building out one of the region's largest Health-Related Social Needs (HRSN) / Social Care Network (SCN) operations. We are seeking a Director of Social Care Network Operations to lead the day-to-day execution, oversight, and growth of this program. This is a build-and-run leadership role: you will own staffing, workflow, network partner relationships, quality and compliance, and financial performance for the program. Responsibilities The Director of Social Care Network Operations is a full-time, on-site role based in New York, NY. This role oversees the design, implementation, and daily management of social care programs and networks that support +MEDRITE patients and communities. Responsibilities include developing operational workflows that integrate social determinants of health into urgent care services, coordinating with internal clinical teams and external community partners, and ensuring continuity of care and appropriate referrals. The Director monitors performance metrics, manages budgets, and optimizes processes to improve access, quality, and patient outcomes. The role also involves leading cross-functional initiatives, training staff on social care protocols, maintaining regulatory and compliance standards, and continuously evaluating opportunities to expand social care outreach and partnerships. Qualifications Clinical expertise in Nursing and Medicine to guide and support integrated social and medical care services. Experience in Case Management with a focus on care coordination, referrals, and patient follow-up across community and clinical resources. Background in Critical Care Medicine or acute care environments to understand urgent care workflows and high-acuity patient needs. Demonstrated Training skills to educate and mentor staff on social care processes, community resources, and patient-centered practices. Proven leadership experience in healthcare operations, preferably in urgent care, ambulatory care, or community health settings. Strong analytical, organizational, and problem-solving abilities to manage complex networks and performance metrics. Excellent communication and interpersonal skills for collaboration with clinical teams, community organizations, and external partners.
